چکیده
Photodynamic therapy (PDT) is used in Europe to treat basal cell carcinoma (BCC), but it not approved the USA due uncertainties about efficacy. Vitamin D3 (VD3; cholecalciferol) treatment prior PDT improves BCC responses mice. A prospective, double-blind, crossover clinical trial [NCT03467789] was designed test whether oral VD3 pretreatment enhances response blue light PDT. Participants received 3 treatments (20% ALA, 4 h; 417 nm, 30 min) 2 months apart. High-dose or placebo administered each of first two sessions. Lesions were recorded with a 3D digital camera allow software-assisted tumor volume analysis. Treatment-resistant tumors biopsied at final visit. To date, 24 patients and 128 BCCs have been analyzed. Two-thirds (70%) all lesions cleared completely after Of 30% that failed clear, except one superficial either nodular, micronodular, adenoid, infiltrative subtypes. assess ability neoadjuvant potentiate efficacy, we evaluated available determine their relative reduction VD3+PDT placebo+PDT. Tumors fulfilled prediction shrinkage (% reduction) would be greater compared placebo+PDT scored as “Yes”. In our analysis, 15 “Yes” 6 “No”. This >2-fold difference provides preliminary evidence augments therapeutic responsiveness for many tumors. may an effective BCC, especially BCC. Oral given represents novel safe combination approach.

BACKGROUND Photodynamic therapy (PDT) is increasingly used as a noninvasive treatment for nodular basal cell carcinoma (BCC), without a sound evidence base. OBJECTIVE To compare topical PDT, with the use of the sensitizer methyl aminolevulinate, and standard excision surgery in nodular BCC. DESIGN Prospective, randomized study. SETTING University dermatology departments. PATIENTS A tota...

Background: Basal Cell Carcinoma (BCC) is the most common type of skin cancer in human beings. Photodynamic therapy (PDT) is a novel therapeutic method which may be regarded as a non-invasive useful alternative for traditional treatments of BCC. This study was designed with the aim of evaluating the primary response of BCC to PDT.Methods: This clinical trial was perform...
متن کاملEnhanced systemic immune reactivity to a Basal cell carcinoma associated antigen following photodynamic therapy.
PURPOSE Numerous preclinical studies have shown that local photodynamic therapy (PDT) of tumors enhances systemic antitumor immunity. However, other than single-case and anecdotal reports, this phenomenon has not been examined following clinical PDT. To determine whether PDT in a clinical setting enhances systemic recognition of tumor cells, we examined whether PDT of basal cell carcinoma resul...
متن کاملPhotodynamic therapy in the treatment of basal cell carcinoma
Photodynamic therapy (PDT) is a medical procedure based on the activation of the molecules of various exogenous or endogenous chemical substances called photosensitizers by a light source emitting radiation of an adequate wavelength, usually situated in the visible spectrum; photosensitizers are chemical compounds bearing the capacity to selectively concentrate in the neoplastic cells. The ener...
متن کاملTreating basal cell carcinoma: has photodynamic therapy come of age?
Photodynamic therapy (PDT) involves the activation of a photosensitizing drug by visible light to produce activated oxygen species which promote tumour destruction. Despite a rising tide of publications in recent years, PDT was first described 100 years ago. Generalized and severe phototoxicity from early crude photosensitizers limited development, although partial purification produced a haema...
